Survey Solutions by ICAHN is dissolving — not transitioning to ICAHN Surveys. Both owners signed the dissolution on September 2, 2026. Nothing signed names a successor.

Survey Solutions is dissolving. Your surveys don’t have to change.
The Survey Solutions by ICAHN joint ownership is being dissolved, effective September 30, 2026 — and you have a choice. The sales process, survey platform, data feeds, and support team behind your program were built and operated by Canopy — and they continue without interruption. To keep your surveys running, sign a new agreement and BAA with Canopy by October 1, 2026. That’s the entire process.

What’s happening — and why your surveys don’t have to change.
For the past 11 years, every Survey Solutions survey has run on systems built, owned, and operated by Canopy: the sales, the sampling, the mail, phone, and text fielding, the in-hospital tablets, the dashboards you log into, and the technical team that fixes things when they break. Canopy prepared the CMS submission file every quarter along the way.
The Survey Solutions joint ownership is being dissolved, effective September 30, 2026. The name is going away. The sales, platform, the operations, and the team are not: they continue directly as Canopy, the same way they’ve run for more than 180 hospitals and clinics.
What does change is paperwork: to keep your surveys running into the fourth quarter, your hospital needs a new agreement and BAA with Canopy — signed by October 1, 2026. That’s the entire transition.
Approved for CMS CAHPS surveys including HCAHPS and OAS CAHPS.
For the record
Survey Solutions by ICAHN
- Is it dissolving?
- Yes.
- Is it transitioning to ICAHN Surveys?
- No.
- Is it jointly owned?
- Yes.
Survey Solutions by ICAHN dissolves on September 30, 2026. On September 2, both of its owners signed an agreement that says “each organization exits the project,” that neither may market “a product named ‘Survey Solutions by ICAHN’” after September 30, and that the platform behind it “will no longer be available for use by ICAHN” after that date. Both parties move forward independently. The agreement names no successor, and each owner is free to contract with any client.
Survey Solutions by ICAHN is jointly owned by ICAHN and AdCo Advertising Agency, which is Canopy. The master agreement between the two organizations says it in one sentence: “both parties jointly own the product, Survey Solutions by ICAHN.” The Exit Agreement quotes that sentence in full, and each owner leaves with what it brought to the project. For AdCo, that is the platform you log into today, the systems that turn your discharge data into surveys and results, the tablets, the texts, and the reports.
Survey Solutions is not being absorbed by either owner, ICAHN or AdCo. As we have said, every facility has a choice to make for October 1: ICAHN Surveys, Canopy, which is the platform you are on today, or a separate vendor entirely. We are confident enough in our product to let you choose what is right for you.

The Survey Solutions Transition:
What It Means for Your Surveys.
Recorded Wednesday, September 9, 2026
Jake and Julie walked through what’s happening, what the October 1 deadline means, and took questions live. Watch the full recording whenever it suits you — and bring anything it doesn’t answer straight to Jake.
Three dates that matter.
October 1, 2026
Agreement + BAA signed
Q4 begins. CMS only allows vendor changes at quarter boundaries, so this is the one deadline that can't move. With Canopy: two signatures, done.
November 8, 2026
Q4 survey data deadline
Surveys must be initiated within 42 days of discharge — November 12 for an October 1 discharge. Agreements and data must be received by this date.
January 13, 2027
Q3 submission complete
Your July–September surveys finish under the Survey Solutions name, on the same systems as today, and the submission file is prepared on schedule — no matter what you decide about Q4.
The timeline, start to finish.
This updates as dates pass — where things stand right now, and what’s next.
-
September 8 – October 1, 2026
Choose your survey vendor
Every client hospital decides who runs its CAHPS surveys going forward. Whoever you choose, a signed agreement and BAA must be in place before the fourth quarter begins.
-
September 30, 2026
The Survey Solutions joint ownership dissolves
The Survey Solutions joint ownership winds down. The sales, platform, data feeds, and people behind your survey program — Canopy’s — keep operating without interruption.
Canopy continues your Q3 data under the Survey Solutions name — fielding and the January submission file carry on as they always have.
-
October 1, 2026
Q4 begins — agreements must be signed
CMS permits survey-vendor changes only at calendar-quarter boundaries, which makes October 1 the deadline that matters. Missing it risks a gap in publicly reported data that can’t be patched mid-quarter.
If you stay with Canopy
Sign a new agreement and a BAA. That’s the entire list.
If you switch vendors
New data interfaces to build, data transfer testing, and staff retraining — normally 4–8 weeks of work, inside this window.
-
November 8, 2026
Q4 survey data deadline
CMS requires every survey to be initiated within 42 days of the patient’s discharge — November 12 for an October 1 discharge — and mail processing takes lead time, so November 8 is the last day paperwork can be complete and still make the first mailing. With Canopy, surveys simply continue on schedule.
-
Through December 2026
Your Q3 surveys finish — same systems, as always
Fielding for July–September discharges completes under the Survey Solutions name, on the same systems as today, no matter which vendor you choose for Q4. Nothing for you to do here.
-
By January 1, 2027

Log in and let CMS know you have chosen Canopy as your official survey vendor.

January 13, 2027
Q3 data submission wraps up
The final Q3 submission file is prepared on schedule, closing out the Survey Solutions era cleanly. From Q4 forward, Canopy submits directly to CMS under its own HCAHPS and OAS CAHPS approvals.

Frequently asked questions.
No. Survey Solutions by ICAHN dissolves on September 30, 2026. The agreement both owners signed on September 2 says each organization exits the project, that neither may market a product named Survey Solutions by ICAHN after that date, and that the platform behind it will no longer be available for use by ICAHN. Both parties move forward independently, and the agreement names no successor. Survey Solutions is not being absorbed by either owner, ICAHN or AdCo. Every facility on it chooses its own vendor for October 1. The agreement excerpts are shown on this page under What’s happening.
Survey Solutions by ICAHN, a joint ownership between Illinois Critical Access Hospital Network (ICAHN) and Canopy d/b/a AdCo Advertising Agency, is being dissolved effective September 30, 2026. The sales process, survey platform, data feeds, and team behind it were built and operated by Canopy, and they continue without interruption — client hospitals simply sign a new agreement and BAA with Canopy to keep their Q4 surveys running. The clause of the master agreement that sets out that joint ownership, section 5.13, is shown under the next question.
Survey Solutions by ICAHN is jointly owned by AdCo Advertising Agency (Canopy) and ICAHN. It began 11 years ago as a joint project to bring quality, affordable patient surveys to rural hospitals. The ownership is written into the master agreement between the two organizations — section 5.13 is shown below.
“Technology subcontractor” is CMS terminology. It describes the role AdCo (Canopy) fills when it fields surveys on behalf of the survey vendor of record, which for these programs has been Survey Solutions by ICAHN. Presented as the only role AdCo has, the label leaves out half the picture: AdCo also jointly owns Survey Solutions by ICAHN — see section 5.13 of the master agreement in the previous answer. Co-owner and CMS subcontractor. Both are true.
This is the one question where you may hear two different answers, so here is ours. Your current contract is signed with Survey Solutions by ICAHN — a jointly owned product that both owners have agreed ends September 30, 2026. These contracts exist to satisfy a federal compliance program, and CMS’s HCAHPS Quality Assurance Guidelines define an active contract as one in which the survey vendor “is authorized by one or more hospital client(s) to submit HCAHPS data.” There should be no grey area about who holds that authorization for your facility after September 30, so on our attorneys’ advice every facility that stays with Canopy signs a new contract and BAA with Canopy. Both owners also agreed in writing that each is free to contract with any existing client, so whatever your current contract says, the choice is yours. Nothing you have already paid is lost: Canopy credits every month you have paid under your current contract. If you are inside a 90-day notice window under that contract, call Jake — that notice provision runs to Survey Solutions by ICAHN, which is dissolving, and he will walk through your dates with you.
Nothing needs to be sent to Canopy. Your current agreement is with Survey Solutions by ICAHN, which both owners have agreed ends September 30, 2026, and its notice provision runs to that product. Both owners also agreed in writing that each is free to contract with any existing client, so whatever your current contract says, the choice is yours. Inside a 90-day notice window, or close to your renewal date? Call Jake first at (309) 692-6790 and he will walk through your dates with you.
Canopy, a d/b/a of AdCo Advertising Agency, is the company that built and operated the Survey Solutions platform end to end for the past 11 years — the sales, sampling, the mail, phone, and text fielding, the dashboards, and the technical support. If you've logged into your dashboard, you've been working with Canopy all along. Canopy holds its own CMS approvals for the HCAHPS and OAS CAHPS surveys.
Yes. Canopy Associates is listed on both the HCAHPS approved vendor list and the OAS CAHPS approved vendor list, holding its own CMS approval for each survey. Both public lists were updated on September 8, 2026, so a check made before that date would not have shown us.
Two signatures: a new services agreement and a business associate agreement (BAA). Your dashboard, logins, data feeds, tablets, survey operations, and support team carry over exactly as they are — no retraining, no IT project. One welcome-and-preferences call covers the few new options, your new upload address, and a new password.
No. Same file, same template, same schedule. The only change is a new upload address and a new password, and both are covered on your welcome-and-preferences call.
Yes. Survey texts keep going out from the same sender numbers your patients have seen for years, and your tablets keep working as they are. No new registrations, no new setup, nothing to reinstall.
CMS permits hospitals to change survey vendors only at calendar-quarter boundaries, and the fourth quarter begins October 1, 2026. Surveys must also be initiated within 42 days of each patient discharge, so whoever runs your Q4 surveys needs a signed agreement and BAA in place by October 1 — with Canopy, no later than November 8.
They finish normally, under the Survey Solutions name and on the same systems as today — fielding for July–September discharges completes through December and the CMS submission file is prepared on schedule, regardless of which vendor you choose for Q4. No action is needed.
With your permission, it stays right where it is. Staying with Canopy means your dashboards keep their full reporting history — trend lines and board reports continue uninterrupted. A new vendor's reporting potentially starts from zero.
Nothing new. Canopy credits every month you've already paid on your current contract, so until your renewal date the only charge is the mailing fees — paid through April 1 means you pay nothing else until April 1. Extend your agreement three or more years past your current term and the extended time is discounted 10%. Six months of Radia 360 (unlimited email campaigns; texting available as an add-on) and Concise are included free.
Talk to Jake — even if you're comparing options. The honest math: a new vendor means a security review, interface builds with your IT team, testing, and staff retraining, which normally takes 4–8 weeks, against a quick signing window. We'd rather walk you through the details than have you caught short.
Call Jake Hanson immediately at (309) 692-6790. The sooner we talk, the more options you have — don't wait.
Two signatures keep your surveys running.
Call Jake, get every question answered, and have the paperwork done this week — with time to spare. Prepaid months carry over in full, extensions of three years or more are 10% off, and Radia 360 and Concise come free for six.
